Alternative Solutions

A Cheshire East based NHS social prescribing service supporting individuals aged 18 years and over improve their health and wellbeing.

Our Cheshire East based Alternative Solutions service delivers a gold standard of expertise through our dedicated Social Prescribing Link Workers, who are integral to the personalised care journey of patients accessing their local GP surgery or primary care services. By focusing on the principle of “What Matters to Me” rather than “What is the Matter with Me,” we empower individuals to address health inequalities and take proactive steps toward better health and wellbeing.

Aligned with the NHSE Workforce Plan, our approach provides GPs with the time to focus on medical needs while our Social Prescribing team addresses the broader social determinants of health. From tackling loneliness and isolation to supporting housing, debt, and employment challenges, we work to enhance lives by providing compassionate, tailored support for every individual.

Alternative Solutions addresses the consequences of non-medical issues on an individual’s life and wellbeing, including:

Lifestyle issues (exercise, diet, sleep) Managing health conditions effectively Social factors (isolation, relationships) Confidence, positivity, hope Personal finances

How does it work?

 

On referral to Alternative Solutions, our clients are assigned an individual Social Prescribing Link Worker who provides tailored support, working as a partner towards improved health and wellbeing.

This can be on the phone, face to face, or within the community at GP surgeries.

The service provides a range of support including managing symptoms, housing support, benefit advice, LGBT advice, debt management and employment advice and guidance.

If you are an individual and think we can help you, speak to your GP practice to be referred to your social prescriber.
